CME Planning Checklist

Joint Provider (your organization) must complete the required activity documentation to qualify for accredited CME designation.

Joint Provider Application Planning Form

This form must be completed and returned to director@uoma.org. The information provided by your organization will allow the UOMA to review the proposed CME activity for possible accredited CME designation.

This form is used to identify and mitigate all potential conflicts of interests that arise from financial relationships with commercial interest(s) of CME planners with control of activity content.

Joint provider needs to evaluate learners' competence, performance, and or patient outcomes after having attended their CME activity.

After the event, the names and credentials of participants and the number of credits awarded to each attendee must be provided for accreditation records.

All joint providers must submit a planning activity application and review fee. In addition, the joint sponsor is charged an overall joint providership fee based on the number of credits for the activity. See Joint Providership Fee Schedule.

​This form needs to be submitted to determine the number of credits allowable for this activity. If changes occur, please resubmit.

The Joint Providership Statement must be included on all materials stating there are CME credits available. The CME Conflict of Interest Disclosures must be made known to the learners.

The certificate is provided to the learner to verify their participation in the activity. When using your own form, be sure to include the CME statement.

Joint Providership for AOA Category 1 Credits. Later, the joint sponsor is also charged for the number of credits designated for the activity.

Joint Providership for AOA Category 1 Credits and AMA PRA Category 1 Credits. Later, the joint provider is also charged for the number of credits designated for the activity.

This form is used to identify and mitigate all potential conflicts of interests that arise from financial relationships with commercial interest(s) of CME presenters, faculty, editors, panelists, content reviewers with control of activity content.

Joint provider needs to evaluate learners' competence, performance, and or patient outcomes after having attended their CME activity.

Complete this budget form to report revenue generated from the activity. This must be provided, so UOMA can report to the AOA for ACCME reports.
